This form should be completed to register your child for entry in September 2020 to any of the Gloucestershire Grammar Schools.

All successfully submitted on-line registration forms will be confirmed by email within a maximum of three days.  As the deadline approaches you should receive a confirmation email within less than twenty-four hours and on the closing day itself within a maximum of one hour.  It is your responsibility as parent / carer to ensure that your child’s on-line registration form is submitted and processed by the deadline 12 noon on Sunday 30 June 2019.  You must contact us before the deadline if you have not received confirmation.


Before you start please make sure you have read the notes on the previous page and that you have followed the instructions and added our email addresses to your “Safe Senders”.


If you have not done this, please stop, go back and follow the instructions otherwise you may not receive our emails which are an integral part of the registration process.


If you have a Proof of Submission (see previous page) but not received a confirmation email after three days it will probably be as a result of not following the instructions above and adding our email addresses to your Safe Senders.  You need to do this first and then click here to send us an email to confirm that our email addresses are now in your Safe Senders list.  We are unable to provide details of how to do this as there are so many different email providers.  You should do a web search or contact your email service provider.


If you have already submitted a form please do NOT submit a second form to correct your mistakes. Duplicate registration forms will be deleted automatically without being read.

Section 2.  Your child’s details.  


In most cases the Legal Surname and the Surname used on a daily basis will be the same.  However, we must use the Legal Surname when processing your registration form.


Eligibility to sit the Entrance Test - Expected Dates of Birth


Your child's Date of Birth will normally be between 1 September 2008 and 31 August 2009.  If your child's Date of Birth lies outside this range you must obtain written consent (letter or email) from each of the schools you list below as share schools.  If you do not have the written consent of a school your registration for that school will not be accepted.  This written consent MUST be obtained BEFORE you submit your on-line form.


The Gloucestershire Grammar Schools’ Entrance Test is targeted at the expected age range for Year 6 children. Sometimes there is a need for schools to assess candidates who are outside the expected age range. In the event that candidates are legitimately "too old" or "too young" for Year 6, their ages are capped when calculating age-standardised scores.


Candidates with a date of birth before the expected date have their age capped because they would be unduly penalised for being in the year group below that for their age, which is likely to be due to complex reasons, or missing school due to illness.


Candidates with a date of birth after the expected date have their age capped because the child would need to be able to work at the level of the rest of their cohort, and so are not inappropriately advantaged by being tested a year early.


It is only in rare and exceptional circumstances where admissions authorities will consent to the testing of over age candidates.

Section 3.  Your child’s permanent address


Start typing your child’s FULL address (eg 44 Market Square Anytown) into the Postcode box. Keep typing until you get a match or pick your address from the drop down list.  The full address will be entered correctly.  Please note the Post Office Address File (PAF) which is used for all address lookups does not usually use any apostrophes.  Do not enter your address manually unless it cannot be found.


If you have a house name which is not registered with the Post Office please use your house number only.  We validate all addresses using the Post Office Address File (PAF).

A Child's Permanent Address (main residence) is the address where the child permanently lives or lives for the majority of the time. Where a child lives with both parents on an equal basis, both parents must agree which of their addresses to use and enter this on the registration form.  Only one address will be used for all communications.

Section 6.  Preferred Test Centre


The Preferred Test Centre is the Grammar School where you would like your child to sit the Entrance Test.  This will usually be your first preference Grammar School.  Please be aware that there may be limited testing places at some Test Centres and your child may need to sit the Entrance Test at an alternative venue.


If a Test Centre reaches its maximum capacity, then ALL spaces at that Test Centre will be allocated by gender and distance i.e. those living closest to the Test Centre will be given priority.  Any overspill of candidates for that Test Centre will be allocated to a gender appropriate Test Centre closest to the child's home address.


Please be aware that your child’s Test Centre will be allocated after the close of registration.


The location of your Test Centre will not affect your child's chances of getting a place at any of the Grammar Schools provided you specify which of the Grammar Schools you wish to share your child's result with at registration.


We always try to keep twins together - so if your twins are not the same sex and you want them to be tested at the same Test Centre please opt for either Pate's Grammar School or The Crypt School as your Preferred Test Centre.


Once your application is submitted changes to your Preferred Test Centre are not permitted. There will be NO EXCEPTIONS.


However, your preference of Test Centre will be allocated providing capacity allows.


Please note girls cannot sit the tests at an "all boys" school and boys cannot sit the test at an "all girls" school.  This is purely for practical reasons - eg availability of adequate toilet facilities.

Section 7.  Sharing your child’s results


Your child's test results can be shared with any of the Gloucestershire Grammar Schools that admits children of that gender.  Boys cannot share their results with a girls only Grammar School and girls cannot share their results with a boys only Grammar School.


Girls and boys can share their results with the two mixed Grammar Schools - Pate's Grammar School and The Crypt School.

Requests for shares are not in preference order.  They are all treated equally.

Section 8.  Pupil Premium and Service Pupil Premium

 

Pupil Premium


If your child is in receipt of the Pupil Premium at their current Primary School please answer Yes.


You must provide written evidence to your allocated Test Centre before your child sits the Entrance Test.


If you live outside of Gloucestershire you will need to request written evidence from your Home Local Authority and ask them to send the evidence to your allocated Test Centre.  The onus is on the Parent / Carer to ensure that the evidence is provided.

For Service Pupil Premium please contact the Admissions Officer of each school you have selected in Section 7.  They will advise you on the school’s over-subscription policy and any evidence you need to submit.  Each school has its own policy so it is essential that you check directly with each school.


The Grammar Schools are not responsible for collecting or requesting the evidence.

Section 9.  Children in Care (Looked After Children)


A Looked After Child is a child who is in Public Care or has previously been in Public Care. (Children previously in Public Care are children who were in Public Care and ceased to be so because they were adopted or became subject to a Residence Order or Special Guardianship Order).


This question is NOT asking you if you look after your child.


If you answer 'Yes' to this question you will need to provide evidence from the current or previous Care Authority or a copy of the Adoption Certificate or appropriate Court Order.



Is your child a "Looked After Child" as defined by the Act?                                            If you leave this blank it will default to No.


If you answer "yes" you will need to provide evidence to your allocated test centre (see notes above).

Section 10.  Special Needs

   

Your allocated Test Centre needs to know if your child has any Special Needs, disabilities (eg visual impairment) or other factors for consideration which may require reasonable adjustments to be made to the testing process.


Does your child have a medical condition (eg diabetes) that the Test Centre needs to be aware of?


If you list any Special Needs in this box you will need to complete an additional form which must be returned to your Preferred Test Centre with any supporting evidence before 12 noon on Sunday 30 June 2019.


Do not include any other information. This section is for Special Needs only.

If you include any information that is not related to Special Needs it will be deleted.

Please note you must submit supporting evidence to your Preferred Test Centre within FIVE days of submitting your on-line form (or 12 noon on 30 June 2019 whichever occurs first). Evidence submitted after 12 noon on 30 June 2019 will not be considered. (Maximum 150 characters - NOT WORDS!)  You must type in this information - do NOT copy and paste from another document.

Section 11.  Sitting Other Entrance Tests - Clashing Dates


Please note - this is NOT an alternative test date simply because Saturday 14th September 2019 is not convenient.


There will be very few, if any, alternative arrangements and only in highly exceptional circumstances supported by evidence.


Your child can only be in one place on Saturday 14 September 2019.  If you have registered your child to sit an Entrance Test on 14 September 2019 outside of Gloucestershire you will need to decide which Entrance Test has priority and where your child will be on that day.  It is for you to decide.  In highly exceptional circumstances you may be offered an alternative date but this will depend on the agreement of all schools involved.  Schools may choose not to offer an alternative date.  If an alternative date is offered it MUST be accepted.  It is not subject to negotiation.


If you choose for your child to sit an Entrance Test other than the Gloucestershire Entrance Test on Saturday 14 September 2019 you must indicate where that is in the box below.  You will also need to provide evidence of registration.  If you do not provide satisfactory proof your registration for the Gloucestershire Entrance Test will be placed ‘on hold’ and will not be processed.

 

Please do not "gamble" that it will be alright.  If your registration is still 'on hold' when the deadline arrives your child will not be registered and will be unable to sit the Gloucestershire Entrance Test.


The following question only needs to be answered if you have or are planning to register your child for an Entrance Test elsewhere that clashes with the Gloucestershire Grammar Schools Entrance Test on Saturday 14 September 2019.


If you decide after you have registered to consider other Entrance Tests which clash and you have not indicated this no alternative arrangements will be considered.  If in doubt delay your registration until you have reached a decision BUT please remember that no exceptions will be made to the deadline of 12 noon on Sunday 30 June 2019.
